The hex shaped dome and four segmented threads are peculiar features found on many insulator styles produced by the Boston Bottle Works.
Some examples of the CD 158 have fully formed inner skirts without embossing. This particular example has a slightly under poured inner skirt, thus there is no embossing present.
Portion of the dome appears to have been frost popped out, and glued back in (red arrows in second picture). There's a very shallow, slice-type chip directly below the bottom center of the glued piece, which goes from the wire groove to the base. It's pinkie nail width at the center, and broadens some at top and bottom. A little flaking at the same area, just below the wire groove. Other side (first photo) displays excellent.
