#208 CD 133. CITY FIRE ALARM. Aqua.
Produced for the fire alarm system in Fall River, Massachusetts. That city is the only confirmed usage of this type insulator, believed to have been manufactured by Brookfield.
Aside form a flash, up from the reverse base (as seen in the second photo), the insulator would grade very near mint.
