#759 CD 133 CITY FIRE ALARM Aqua.
Produced for the fire alarm system in Fall River, Massachusetts. That city is the only confirmed usage of this type insulator, believed to have been manufactured by Brookfield.
A small, shallow base flake is only a little larger than a wooden match-head. Some shallow surface wear/dullness can be seen above and below the "City" embossing.
