Numerous glass block type insulators have been documented. Of the various types known, this is among the rarest. It is smaller than the typical 1000 and 1003 examples.
Small blemishes, including flaking along the sharp edges, which is often found on block style insulators. One end, including the two protruding flanges, were broken away and replaced with casting resin. The repair was done decades ago, and has discolored.
